To enroll your child, please submit the following forms:
1) Summer Programs Application
2) Emergency Information Card
3) Health History Form. Please also submit a copy of your child's
4) health insurance card, and the non-refundable and non-transferable
5) deposit ($100 per session or $50 per week). All applications will be processed on a “first come, first served” basis and will be numbered and dated.
